ABSTRACT

Anatomic variations in the branching pattern of the axillary artery are common and have been studied by many authors. The origin of the anterior circumflex humeral artery is also various and its association with other variations has been focused recently. This study identified the origin of the anterior circumflex humeral artery, the communication between the musculocutaneous and median nerves, and the third head of the biceps brachii muscle, and then their associations were examined. Axillae of 75 cadavers were dissected and these variations were collected. The anterior circumflex humeral artery arose directly from the third parts of the axillary artery in 82.0% of upper limbs. It also arose as common stem for both humeral arteries in 7.3% of the third part of the axillary artery and in 5.3% of the subscapular artery, respectively. The anterior circumflex humeral artery was absent in 2.7% and two anterior circumflex humeral artery was found in 1.3% of limbs. The communication between the musculocutaneous and median nerves and the third head of the biceps brachii muscle were found in 32.0% and 16.0% of upper limbs, respectively. These variations were not statistically associated with each other. These results may provide deep knowledge about the anatomy of the anterior circumflex humeral artery and contribute to explain its clinical implications.

ABSTRACT

For this study, medical students, intern physicians, and resident physicians, were surveyed about the application and effectiveness for medical procedure training using cadaver. In this survey, with a target population of 170 individuals, 105 individuals (61.7%) showed positive responses to learning medical procedural using cadaver. Resident physicians group (80.8%) showed relatively positive responses compared to 4(th) year medical students (58.1%) and intern physicians (47.6%); however, this did not account for any significant statistical difference. Regarding the application of cadaver to medical procedural practices, 95 individuals (62.1%) showed positive responses. Resident physicians (74.5%) had the highest percentage of positive responses compared to intern physicians (56.7%) and 4(th) year medical students (56.6%), and there was a significant statistical difference. Suture (15.7%) was ranked first as the most suitable medical procedural to be practiced on cadaver; tracheotomy and intubation came second and third respectively. This research confirmed the application and the effectiveness of cadaver for medical procedural training and the improvement of general medical procedural ability are expected if the cadaver is applied not only to medical schools but also to intern and resident physician training.

ABSTRACT

The axillary artery (AA) is often referred to as having three parts, with these divisions based on its location relative to the pectoralis minor muscle. In third part, AA gives off the subscapular (SSA), anterior circumflex humeral, and posterior circumflex humeral arteries (PCHA). However, variations in these arteries were extremely diverse. So, we observed actually some branching patterns of these arteries in this study. METHOD: We studied the pattern of SSA in 128 upper limbs from donated cadavers. RESULT: SSA was originated directly from the third and second parts of AA in 37.5% (48/128) and 4.7% (6/128), respectively. A PCHA made a common trunk with SSA in 25.8% (33/128), and these trunks arose from the third and second parts of AA in 21.1% (27/128) and 4.7% (6/128), respectively. A lateral thoracic artery (LTA) arose from SSA in 12.5% (16/128), and these were originated from the third and second parts of AA in 4.7% (6/128) and 7.8% (10/128), respectively. In 19.5% (25/128) of upper limbs, LTA, SSA, and PCHA have a common trunk, and these arose from the third and second parts of AA in 12.5% (16/128) and 7.0% (9/128), respectively. According to the branching pattern of the SSA, its origin was significantly different.

ABSTRACT

During an educational dissection, accessory tendon of the extensor pollicis brevis muscle was found on the left side in a Korean cadaver. The abductor pollicis longus, extensor pollicis brevis, and extensor pollicis longus muscles showed normal morphology and course: however, narrow muscle belly originated between the extensor pollicis brevis and extensor pollicis longus muscles. It crossed the anatomical snuff box and then inserted on the base of the distal phalanx of the thumb. The author describes this previously novel case report and discusses the clinical implications of such a variant.

ABSTRACT

PURPOSE: Green tea is known as a potent anti-oxidant, anti-carcinogen, and genetic protector. Glyphosate (N-phosphonomethyl glycine) is a widely used non-selective herbicide that causes DNA damage. The present study was conducted to investigate the protective effects of green tea in human blood lymphocytes exposed to glyphosate using the Sister Chromatid Exchange (SCE) frequency method. METHODS: Peripheral blood was obtained from 10 volunteers and cultured through four different conditions. Four groups were divided into control, glyphosate only (300 ng/mL), glyphosate and low (20 µm) concentrations of epigallocatechin gallate (EGCG) and glyphosate and high (100 µm) concentrations of EGCG. RESULTS: The glyphosate exposed groups had a higher mean SCE frequency (10.33±2.50) than the control group (6.38±2.28, p<0.001). The low concentrations of EGCG groups had a lower mean SCE frequency (9.91±1.93) than the glyphosate-only group, although this difference was not significant (p=0.219). However, the high concentration group (9.49±1.85) had a significantly lower SCE frequency than the glyphosate-only group (p=0.001). CONCLUSION: EGCG has a gene protective effect in human lymphocytes exposed to the genotoxicity of glyphosate in the case of high concentrations.

ABSTRACT

PURPOSE: Glyphosate is a widely used non-selective herbicide. Previous studies have shown that glyphosate has genotoxicity, and that even low-doses of glyphosate can cause DNA damage. Melatonin is a hormone produced and secreted by the pineal gland that is known to be a potent anti-carcinogen, anti-oxidant, and genetic protector. This study was conducted to investigate the genoprotective effect of melatonin against glyphosate in human blood lymphocytes. METHODS: Human peripheral blood was obtained from 15 young, healthy volunteers and cultured under four different toxicologic conditions. The four groups consisted of a control group, glyphosate only group (300 ng/mL), glyphosate with low level of melatonin group (50 µM), and glyphosate with high level of melatonin group (200 µM). The mean Sister Chromatid Exchange (SCE) frequency of each group was then analyzed. RESULTS: Glyphosate exposed groups had a higher mean SCE frequency (10.33±2.50) than the control group (6.78±2.31, p<0.001). Interestingly, the group that received a low-level of melatonin had a lower mean SCE frequency (8.67±2.58) than the glyphosate-only group, while the group that received a high level of melatonin had a much lower mean SCE frequency (8.06±2.50) than the glyphosate-only group. There was statistical significance. CONCLUSION: Melatonin exerted a potent gene protective effect against the genotoxicity of glyphosate on human blood lymphocytes in a dose-dependent fashion.

ABSTRACT

Continuous attention has been developed on the anatomical variations of the axilla in anatomist and surgeon due to their clinical importance. The axillary region is an anatomical space between the lateral part of the chest wall and the medial aspect of the upper limb. During the routine dissection of embalmed cadavers, we found variant muscular slip originating from the lateral border of tendinous part of the latissimus dorsi and continuing 9 cm more crossing the axilla. And then, it inserted into the superior margin of the insertion of the pectoralis major. We considered this muscular variation as axillary arch muscle. Correct identification of the relevant anatomy and subsequent simple surgical division is curative, paying special attention to anatomical variations in this region and its clinical importance due to its close relationship to the neurovascular elements of the axilla.

ABSTRACT

The purpose of this study was to report variations of the cubital superficial vein patterns in the Korean subjects, which was investigated by using venous illuminator, AccuVein. The 200 Korean subjects were randomly chosen from the patients and staff of the Keimyung University Dongsan Medical Center in Daegu, Korea. After excluding the inappropriate cases for detecting venous pattern, we collected 174 cases of right upper limbs and 179 cases of left upper limbs. The superficial veins of the cubital fossa were detected and classified into four types according to the presence of the median cubital vein (MCV) or median antebrachial vein. The type II, presenting the both cephalic and basilic vein connected by the MCV, was most common (177 upper limbs, 50.1%). Although the most common type in male and female was different as type I (108 upper limbs, 49.3%) and type II (75 upper limbs, 56.0%), respectively, statistical significance was not detected (P=0.241). The frequency of the each types between right and left upper limbs was also not different (P=0.973). Among 154 subjects who were observed the venous pattern in the both upper limbs, 76 subjects (49.3%) had the same venous pattern. Using AccuVein to investigate the venous pattern has an advantage of lager scale examination compared to the cadaver study. Our results might be helpful for medical practitioner to be aware of the variation of the superficial cubital superficial vein.

ABSTRACT

PURPOSE: Glyphosate (N-phosphonomethyl glycine) is widely used as an herbicide for weed control in rural areas. It is also readily available for suicide attempts. Glyphosate has high toxicity and negatively affects the human body. The aim of this investigation was to study the genotoxicity of a low-concentration of glyphosate through sister chromatid exchange (SCE) in human blood lymphocytes in vitro. METHODS: Primary lymphocyte cultures were obtained from blood samples of 11 males and seven females who had been exposed to glyphosate (0, 100, 200, and 300 ng/mL). The frequency of SCEs was examined and statistical analysis was performed. RESULTS: All doses of glyphosate induced a significant dose-dependent increase in SCE frequency compared with the control group (P<0.001). In particular, the SCE frequency for exposure to low-dose glyphosate was significantly higher in females than in males. CONCLUSION: According to the result of this study, even a low-dose of glyphosate may damage DNA and females are more vulnerable to glyphosate.

ABSTRACT

The palmaris longus (PL) is a slender, spindle-shaped weak flexor of the wrist. Congenital absence of the PL is estimated to occur in 15% among individuals worldwide. However, the frequency of its absence varies considerably among different population groups and with different detection techniques. In the present study, the presence of the PL tendon was examined in a Korean population (n=269) using three clinical tests, namely the Traditional Test, Mishra's Test II, and the Gangata Test. We classified subjects into six types based on whether inspection or palpation was required to determine the presence of the PL and flexor carpi radialis. The most reliable test was determined using Kendall's coefficient of concordance. Our results showed that the PL tendon was absent in 4.1% of the subjects in our study, and bilateral and unilateral absences were 2.2% and 1.8%, respectively. Statistical analysis revealed that these tests had similar reliability for assessing the PL tendon, and the Traditional Test showed the highest effectiveness, at 93%. Therefore the Traditional Test was found to be the most effective for revealing the PL in this Korean population.

ABSTRACT

The renal veins drain the kidney into the inferior vena cava and unite in a variable fashion to form the renal vein. The left renal vein is normally located in front of the aorta. However, the retro-aortic renal vein may course posterior to the aorta due to embryological developmental anomalies. During educational dissection, a rare variation of the left renal vein was found in a 66-year old male cadaver. The double retro-aortic renal veins coursed behind the aorta to drain into the inferior vena cava. The superior retro-aortic renal vein drained into the inferior vena cava at the lower border of the L2 vertebra, and the inferior retro-aortic renal vein drained into the inferior vena cava at the upper border of the L4 vertebra. Such a variant is rare, and is a clinically important observation which should be noted by vascular surgeons, oncologists, and traumatologists.

ABSTRACT

The digastric muscle, as the landmark in head and neck surgery, has two bellies, of which various variations have been reported. In the submental region of a 72-year-old Korean male cadaver, bilateral variations were found in the anterior belly of the digastric muscle. Two accessory bellies, medial to the two normal anterior bellies of the digastric muscle, ran posterior and medially, merging and attaching at the mylohyoid raphe of the mylohyoid muscle. The 3rd accessory belly originated from the right intermediate tendon and ran horizontally, merging the right lower bundle of the right accessory belly and inserted together. These accessory bellies had no connection with the left anterior belly. This unique variation has not been reported in the literature previously, and this presentation will guide clinicians during surgical interventions and radiological diagnoses.

ABSTRACT

Arterial variations in left upper limb were observed in an embalmed cadaver. The second part of the axillary artery divided into two stems, medial and lateral stems. The lateral stem was deeper than the medial one and the median nerve was located between these two stems. The lateral stem divided into five branches: the subscapular, two anterior circumflex humeral, a posterior circumflex humeral, and profunda brachii arteries. The medial stem adopted its course superficial to the median nerve, and then divided into the ulnar and radial arteries at elbow level. The present authors describe a previously unreported case and discuss the clinical implications of such a variant.

ABSTRACT

Anatomical variation in the abdominal aorta is common and extremely important. We found a rare variation of the abdominal aorta in a Korean male cadaver during a routine dissection course. The superior mesenteric artery gives off a small branch for the descending colon, named accessory left colic artery due to its distribution. The inferior mesenteric artery arises normally from the abdominal aorta and supplied the sigmoid colon and the upper part of the rectum. This variation was rare and it has not been reported previously in Korean. Knowledge of the arterial variation in this region is considerable important during abdominal surgery and invasive procedures. The authors describe this previously unreported case and discuss the clinical implications of such a variant with a brief review of the literature.

ABSTRACT

The Wiskott-Aldrich syndrome (WAS) is a severe X-linked disorder characterized classically by thrombocytopenia, immunodeficiency, and eczema. The phenotype observed in this syndrome is caused by mutation in the WAS gene. Peripheral blood DNAs were isolated from an 18-month-old boy with WAS and his mother, maternal uncle, and maternal grandmother. Genetic analysis for the detection of a mutation of WAS gene was performed by polymerase chain reaction-single strand conformational polymorphism analysis (PCR-SSCP) and direct sequencing of the PCR product. In PCR-SSCP, the patient and his maternal uncle had an abnormal shift band, which was not found in normal controls, and his mother and maternal grandmother showed heterozygous bands. In direct sequencing analysis, the patient with WAS had CGC-->CAC point mutation in exon 2 that resulted in an amino acid change in codon 86 (Arg86His). The present study identified a gene mutation responsible for WAS at a mutation hotspot of the WAS gene in a Korean family.

ABSTRACT

Generally, many Asian women tend to dislike the square jaw, as they believe it makes the face look wider, giving a stubborn and strong impression. Contouring of the mandible is therefore a relatively common aesthetic procedure among Asians. These days, the use of botulinum toxin for contouring of the lower face offer simple alternative to surgery. Motor point, which is the site over a muscle where its contraction may be elicited by a minimal intensity short duration electrical stimulus, is the optimal injection point of botulinum toxin. Study was undertaken to identify the location of motor point of the masseter muscle and the skin surface landmark. First, the thickest point of the masseter muscle was inspected through palpation and inspection by 3 different individual plastic surgeons and then compound muscle action potentials(CMAPs) of masseter muscle in 15 health volunteers were recorded using EMG. For the localization of the measured points, line between lateral canthus to the mandibular angle was used. Location of motor points were mapped to skin surface from lateral canthus in a percentage of the distance along the landmark line and in distance in millimeters. The clinical injection point was located at 71.69 percentile and 7.3mm of the landmark line. The motor point test was located at 72.54 percentile and 7.1mm of the landmark line. The depth of motor point was 16mm. There was no statistically significant difference between the clinical injection point and the motor point. We conclude that surface mapping of motor point of the masseter muscle would increase accessibility and accuracy in botulinum toxin injection for contouring of the lower face.
